Position Description
Medix is now hiring a field-based Case Management Nurse for a Hospice agency in Palm Springs!
Type: Direct Hire/Permanent
Setting: Field Based, mileage reimbursement provided at the federal rate
Shift: Mon-Fri (8am-5pm)
Requirements:
- Valid Registered Nurse License (California)
- Must be local to the Palm Springs area or surrounding areas
- Good driving record in addition to a valid Driver's License and Auto Insurance
- Must have BSN or ADN with one year of clinical experience
Duties:
- Manage and maintain a caseload of up to 12 patients, providing individualized attention and care.
- Coordinate all aspects of patient and family care, including services provided by the interdisciplinary team, social worker, chaplain, home health aide, volunteers, and physician.
- Collaborate with contract personnel such as Physical Therapy, Occupational Therapy, Dietary, Speech Therapy, and Pharmacy to ensure optimal patient care.
- Participate in Interdisciplinary Team Meetings as required to discuss and update patient care plans.
- Regularly update and integrate care plans to reflect changes in patient condition.
- Provide skilled nursing care as outlined in the interdisciplinary plan of care.
- Ensure and document appropriate use of medications, durable medical equipment (DME), and medical supplies.
- Conduct on-site evaluations of Certified Home Health Aides (CHHAs) and volunteers.
- Educate patients, families, and caregivers on care plans, medication management, and available support services.
- Clearly communicate how and when to contact the Nurse Case Manager under normal circumstances or during emergencies.
- Participate in the on-call rotation, including weekends and holidays, if the need arises.
- Perform other related duties as assigned.
